If you have never volunteered with us before, we'll need you to fill out the following forms in order to enter the prison. The first form may be filled out online and the rest are in PDF format. Please print the PDF forms and fax or mail it back to us.

Long Application - For security reasons, we will call you to get a couple of pieces of information.

John Roach Release Form - For those who are joining us at John R. Roach Juvenile Detention Center. This must be signed and mailed to us at least one week before visitation.

F.A.S.T. Form - It is a departmental policy for all volunteers to submit to a criminal background check and submit to the Fingerprint Applicants of Texas (FAST) Services. You can schedule a fingerprint appointment by visiting www.ibtfingerprint.com or by calling 1-888-467-2080. The cost for fingerprinting is $9.95 and you may pay for FAST services online with a credit card or onsite with a check or money order only. Please remember to take the FAST Pass with you and have section two of the F.A.S.T. form completed for your scheduled appointment. Upon completion of your fingerprint process, FAST services will notify juvenile detention personnel of your completed application.
